Washington has two features that change this question, and neither is about the carrier.

The first is that Washington runs its own health insurance exchange rather than using the federal one. The federal listing of state-based exchanges for plan year 2026 names Washington and its marketplace, Washington Health Plan Finder [3]. The second is geography: our Washington campuses sit at the far south of the state, inside the Portland metropolitan area, which makes the state line a live issue rather than a formality.

What is actually covered comes down to the policy in your hand, and we publish nothing about it. One call to our admissions team for a benefits check beats an afternoon of reading. If you want the carrier-neutral version of all this first, start with whether insurance covers rehab.

UnitedHealthcare in Washington: Plan Types Sold Here

Start by identifying which product you hold, because the rules and the phone queue both differ.

An employer group plan comes through work. An individual plan bought for yourself comes through Washington Health Plan Finder, the state’s own exchange [3]. Apple Health is Washington’s Medicaid program and is a separate product even where a familiar company name appears on the card.

The state-exchange point has a practical edge to it. Enrollment, special enrollment periods and appeals for an individual plan run through Washington’s own marketplace rather than through HealthCare.gov. If someone has sent you to the federal site for a Washington individual plan, that is the wrong door.

Which product you hold also determines who you are arguing with if a decision goes against you. Check the card before the call. Our page on what rehab costs covers what an unresolved gap looks like in practice.

What a Plan Has to Cover, and What Authorization Turns On

The floor here is federal, and it is firmer than most people assume.

Every plan sold on a marketplace treats this care as an essential health benefit, which the federal consumer guidance spells out as three things a plan must include: counseling and psychotherapy, inpatient behavioral health stays, and treatment for a substance use disorder [2]. Refusing someone, or pricing them differently, because of a pre-existing condition is barred, and a substance use disorder counts as one [2]. So is putting an annual or lifetime dollar ceiling on an essential health benefit [2]. Within those rules, the detail varies by state and by the plan chosen [2].

Parity is the second layer. Under the Mental Health Parity and Addiction Equity Act, a plan covering mental health and substance use care may not apply harsher limits to it than to medical and surgical care, and that reaches prior authorization and step therapy as well as visit and day caps [1]. The scope is worth knowing: it applies to non-federal governmental plans with more than 50 employees, to group health plans of private employers with more than 50 employees, and to the individual market [1]. Plans from small employers are not covered directly; they get comparable protection through the Affordable Care Act’s essential health benefits requirement.

What neither layer does is decide your case. Authorization turns on documented clinical need from an assessment, which is why the assessment comes first and the phone call second.

You will not find any carrier’s own review criteria described on this site. Describing them would mean citing a regulator who has published them, and no such publication is available to us here.

Settling the coverage question before admission in southwest Washington

The Recovery Village Ridgefield is in Ridgefield, Washington, and its own page publishes medical detox, inpatient treatment, partial hospitalization programming, intensive outpatient programming, dual diagnosis treatment and medically-assisted treatment. Our detox center in Vancouver, Washington is about 11 miles from Portland in a straight line, and farther by road.

Our Washington Campuses, and Which Levels Each One Carries

Two sites, and the difference between them decides which one you call.

The Recovery Village Ridgefield publishes medical detox, inpatient treatment, partial hospitalization programming, intensive outpatient programming, dual diagnosis treatment and medically-assisted treatment on its own page [5]. That is close to the full ladder on one site, which matters when the plan is likely to step down rather than end.

The Recovery Village Ridgefield Detox Center in Vancouver is the second site, at 5114 NE 94th Avenue. Its own page publishes 16 detox beds and 24-hour care, and describes a timely transition to inpatient rehab at the Ridgefield location afterwards [6]. It is the closer of the two to Portland: about 11 miles in a straight line against roughly 20 miles to Ridgefield, and farther than either by road.

On carriers, the campus speaks for itself and we leave the sentence as it wrote it. Its page states that The Recovery Village Ridgefield is in-network with most major insurance carriers including Aetna, Blue Cross Blue Shield, Cigna, Humana, and United [5]. Treat that as a description of the campus, not a ruling on a policy. Call to verify coverage regardless.

Our explainer on levels of care sets out the tiers, and medical detox describes the most supervised one.

Crossing the Columbia: Networks, State Lines and the Portland Metro

This is the part that is specific to here, and it catches people out every year.

Vancouver and Ridgefield are in Washington. Portland is in Oregon. The river between them is a state line for insurance purposes even though it is a commute for everything else. A plan bought on Washington’s exchange and a plan bought for Oregon are regulated by different states, sold by different rosters and built on different networks.

Three consequences follow. An Oregon resident looking at a Washington campus is looking at an out-of-state facility, whatever the map says. A Washington resident with an individual plan bought through the state exchange has a network drawn for Washington. And an employer plan may behave differently from either, because large group networks often span both states.

None of that makes the crossing impossible. It makes it something to settle first. Two questions do the work: is this facility in network for my product, and if not, will you authorize it as an exception because nothing in network offers this level of care nearby. The second question is asked far less often than it should be. Our directory of treatment centers shows which of our campuses sits in which state.

Prior Authorization and Continued Stay Review, Step by Step

The sequence matters more than the paperwork, and most families meet it in the wrong order.

An assessment comes first and produces a recommended level of care with clinical reasoning attached. That reasoning is the evidence everything downstream rests on. The facility then seeks authorization ahead of admission, or inside whatever window the plan allows for an urgent one.

Continued stay review is where expectations break. An authorization typically covers a period rather than an episode, and it is revisited. A step down is a decision about documented criteria on a date, not a verdict on how hard anyone is trying.

Hold on to two things. Parity reaches prior authorization and step therapy, so a tougher process here than for medical and surgical care is precisely what the statute restrains [1]. And no denial is final on the day it arrives. Get the stated reason on paper. Find out which criteria were used. Then have the treating clinician request a peer-to-peer review, which is a conversation between two doctors rather than a form. Our page on how admission works lays out the sequence.

If UnitedHealthcare Is Not the Right Route: Apple Health, Marketplace and Self-Pay

There is more than one door, and the commercial plan is not always the one that opens.

Apple Health is Washington’s Medicaid program, and its managed care is run by the Washington State Health Care Authority [4]. Whether you qualify turns on income, household and circumstances, and it is worth applying rather than assuming. The Health Care Authority is the agency to ask about Apple Health substance use treatment benefits. It says an Apple Health managed care plan coordinates physical health, mental health and substance use disorder treatment services under one health plan [4].

An individual marketplace plan is the second door, bought through Washington Health Plan Finder as the state’s own exchange [3], and carrying the essential health benefit protections above [2].

Self-pay is the third. The figure follows how intensive the care is rather than how many weeks it lasts, and it is worth asking about even when you expect the answer to be impossible. Our pages on inpatient rehab and outpatient care describe what the money is buying at each end.

What the Benefits Verification Call Covers

Arrive with information rather than worries and the call shortens dramatically.

Four items should be in front of you: the card itself, with the member ID and group number; the level of care a clinician recommended, quoted as they said it; who assessed you and on what date; and any submission already made on your behalf.

Then work through a list of your own. Is this tier a benefit under the policy. Does it need approval before admission. How much of the deductible and the out-of-pocket maximum is already spent. Does this particular product treat the facility as in network. And how frequently will the stay be re-reviewed once it starts.

Close by asking for a reference number and a written confirmation. Telephone reassurance is not a coverage determination, and it will not be remembered when the statement arrives. Our guide to keeping your job during rehab deals with the worry most people leave unspoken.

Frequently Asked Questions

Does UnitedHealthcare cover rehab in Washington?

The document in your hand decides that, and no page here claims otherwise about any policy. Two things are fixed by law. Any marketplace plan treats treatment for a substance use disorder as an essential health benefit. And parity forbids harsher restrictions here than on medical and surgical care, approval requirements and step therapy included. A benefits check turns law into a specific answer.

Is Recovery Village Ridgefield in network with UnitedHealthcare?

The campus answers that, and we leave its wording alone. Its own page names Aetna, Blue Cross Blue Shield, Cigna, Humana and United among the carriers The Recovery Village Ridgefield is in network with. That describes the campus in general terms. It settles nothing about your particular product, because network status differs between product lines inside one company. Verify before relying on it.

What does UnitedHealthcare cover for detox in Washington?

No carrier’s review criteria appear here; setting them out would require a regulator who has published them, and we have none. Verifiable instead: withdrawal management sits inside the substance use benefit that every marketplace plan must include, and approval normally follows documented clinical need established at an assessment. Put the criteria question to the plan itself and insist on a written reply.

Can I use a Washington UnitedHealthcare plan at an out-of-state campus?

Possibly, though network rules govern this rather than distance. It bites harder here than almost anywhere, since the Portland metropolitan area sits across a state boundary and a twenty-minute journey can still count as leaving the state. Establish whether this product counts the facility as in network, then ask about an exception if nothing in network offers the tier you need.

How do I verify my benefits?

Either ring the number printed on your card or hand the job to our admissions team. Gather the card, the tier a clinician recommended, and who assessed you on what date. Then establish five things: is the tier a benefit, is approval needed before admission, how much deductible remains this year, does this product count the facility as in network, and how often the stay gets re-reviewed. Finish with a reference number.
